The Tsunayashiro family already performed experiments related to organ-harvesting since long ago. Ikomikidomoe already robbed Hikone of some of the Rei-o fragments, so why go straight for the head? I say this because Ikomikidomoe immediately tried to tear off Hikone's head once separated from him. I think that the scene with Ikomikidomoe and Hikone pretty much confirms (at least, for the sake of fairness, I suppose I should say, 'heavily implies') that Gremmy is indeed the Rei-o's brain.
